About Case Management

Case Management services include planning, linking, advocating, coordinating and monitoring to assist individuals in gaining access to needed services, financial assistance, housing, employment, education, social services and other services and supports identified through the person-centered planning process.

What we do

CDS Case Managers provide a holistic approach to supports by: 

  • Assuring the provision of person centered planning
  • Providing linkages to community resources for financial services, housing, employment, education and social services
  • Coordinating services and supports
  • Assisting individuals with maintaining and maximizing sources of financial support including: Supplemental Security Income, Social Security Disability Insurance, home help and food assistance
  • Addressing health and safety issues
  • Monitoring of public benefits

Funding for Case Management

The first step to receiving services is to contact the Integrated Services of Kalamazoo Access Center at (269) 553-8060. The staff at the Access Center will do an initial screening to determine what services you qualify for and connect you to those services. 

Cost for services: Fees are based upon your ability to pay or your health insurance coverage. While no one is denied services based on their financial situation, there may be a waiting list in place to receive non-Medicaid covered services

Getting Started

Upon approval of services through ISK, WMU Center for Disability Services will be a choice of Case Management providers. If you choose, please inform the intake clinician of your preference for WMU Center for Disability Services Case Management.
