Student Affiliation Agreement Review Checklist

Please complete the checklist to assist this office with legal review of your proposed affiliation agreement. Please complete both the checklist and the proposed agreement. You should receive a confirmation of receipt via email shortly after sending this information to us; if you do not, please follow up by calling our office at (269) 387-1900.

Please note that, given competing deadlines and other workflow considerations, the sooner you send us an agreement for review before any mandated deadlines, the better. If you have not received a substantive response within two weeks of submission, please contact our office to inquire as to the status of your agreement.

If it is specific to WMU, it must state “EXCEPT AS REQUIRED BY LAW” or otherwise allow disclosure pursuant to FIOA.
You must either renegotiate the agreement or insert them. If the agreement is of indefinite duration, it must contain a re-review date. Maximum time between reviews is five years.
Keep any such statement in your file.
Business Services Manager will need to approve.
This language is required.
Students must separately sign an authorization and waiver form that should be kept on file with the completed, signed agreement. Contact Office of General Counsel for assistance if necessary.
You must either add this language or renegotiate to include it.
It must be reviewed by the Office of the General Counsel.
