Contact Us is the bottom link in column one and is required on nearly all sections. The link should go to a standard format PHP form, which may be modified to request a variety of information from the sender. In the WMU content management system, the Contact Us form is created using the Webform content type.
Senders can, for example, be asked to identify themselves as prospective students, students, alumni, parents or family members, faculty or staff, or emeriti and retirees. This kind of information may be helpful in responding to questions, but only request information that will actually be used, and only require information that is essential.
The simpler and shorter the form, the better. Simple forms include fields for name, email address, phone, subject and message; and phone is usually optional.
The output from the form is sent as an email message to one or more addresses. Ideal for this purpose is an office email address to which multiple staff members have access, and which is checked at least once each business day. Alternately, the messages may go to two or three individual staff email addresses, with one person assigned primary responsibility, and the others available to cover.
Additional contact information should be listed at the bottom of the form. It should minimally include a mailing address and main office phone number, formatted exactly as shown below.

Other information can include physical location and campus mail stop (see above), secondary phone numbers for major divisions within the office or department, and when critical, general office hours. Remember, the more you include, the more you need to maintain.
