- An official transcript from each undergraduate and graduate institution you have attended except for WMU

The Departmental Information Form (DIF)
- A one (1) page biographical essay
- A copy of your current resume
- An official transcript from each undergraduate and graduate institution you have attended except for WMU
- Letter of recommendation to the admissions committee (optional, but recommended)
