INFORMED CONSENT DIRECTIONS

 

In conducting this survey, Federal Law requires that the right to and protection of the privacy and choice of students to participate must be observed.  Therefore, the accompanying informed consent forms should be sent home with the students or mailed to their homes approximately one week prior to the survey date.  Parents who do not wish their children to participate in the survey will sign their Informed Consent Form and return it to the school prior to the survey date. The return of this form will allow you to organize an alternative activity for the non-participating students.

 

Additionally, the students themselves may also decline participation at the time of the survey. The "self-excused" student probably should be escorted to the alternative activity site and join those excused via the returned Informed Consent Forms. Remember, neither you nor we are permitted to pressure students to participate in the survey in any way.

 

Non-participants have the same right to anonymity as participants. To maintain the highest level of anonymity possible, we have two requests.  Please keep to a minimum the number of staff members that come in contact with the students who choose not to participate via the consent form and those who decline on site.  While we do need to know the total number of those declining to participate, please destroy all lists of non-participants and the signed informed consent forms returned to you after the survey.

 

Following these procedures about non-participants will help the School System and the Michigan AOD School Survey maintain promised professional and ethical standards.
