Filing Insurance Claims

Participating Insurances Plans

If you would like Sindecuse to bill your insurance, download and complete the Patient Insurance Information form in advance and bring it, as well as your current insurance and pharmacy card(s), to your appointment. We will submit claims to all participating insurance plans and the charges will not be transferred to your University student account until an Explanation of Benefits (EOB) is received from your insurance company. After receiving the EOB, any amounts designated by your plan as patient responsibility, such as co-pays, deductibles, or non-covered services, supplies or braces, will be transferred to your University account.

If you do not have your insurance information with you at the time of your visit, you may bring it back within 14 days and Sindecuse will submit your claim. However, if your insurance information is not received within 14 days from the day of service, charges will be placed on your University student account.

For specific plan coverage and reimbursement information, it is best to call your insurance carrier using the phone number on the back of your insurance card.

Non-Participating Insurance Plans

As a courtesy, for insurance companies originating in the U.S. and Canada of which Sindecuse does not participate, a claim will be submitted to your primary insurance plan (courtesy billing), provided correct and complete policy information is on file with us. Reimbursement should be sent to the subscriber* of the policy. Charges will be placed on your University student account and you are responsible for payment.

Download & complete the Patient Insurance Information form and bring this completed form and your current health insurance and pharmacy insurance cards to your appointment. HMO insurances usually require the patient to obtain approval prior to services being performed. For specific plan coverage and reimbursement information, it is best to call your insurance carrier using the phone number on the back of your insurance card.

If you do not have any insurance coverage, you may be interested in purchasing coverage provided through Chickering/Aetna, the WMU Sponsored University Student Health Insurance Plan.

*Subscriber is the primary holder of the policy, often a parent or spouse who obtains coverage through their employer.

All Patients:

Some laboratory services, such as cultures and pap smears, are sent to Borgess Laboratory. If provided at the time of your visit, Sindecuse sends your insurance information to Borgess and they bill accordingly. You may receive a separate bill from Borgess depending upon their participation with various insurance companies.
