Request access forms:
Requesting Security Access Process for obtaining access to any of the secure student systems
SIS (Banner) request for academic access Only for depts with the Registrar's Office as their security officer (available through GoWMU)
SIS (Banner) request for the business office For business office employees to enter/view accounts data (available through GoWMU)
Forms standard throughout the University are:
Cognos account request form: Web-based reporting system for data in WMU’s Student Information System (available through GoWMU)
Image Now request for access: Access to the scanned student information (available through GoWMU)
SSN access request form: Access to the field with the social security number limited to personnel for whom access is a business necessity (available through GoWMU)

As a user with privileges to a secure student information system, you have access to data pertaining to WMU faculty, staff, and students. As a user entrusted with access, you are to become familiar with and agree to abide by these policies.
All data viewed, modified, or retrieved via a University data system is ONLY for the use of University faculty and staff doing the work of the University. Any other use or disclosure of this information is strictly prohibited.

Family Education Rights and Privacy Act (FERPA):
Federal law that governs and protects students' rights to their individual education records. wmich.edu/registrar/FERPA
Social Security Number protection policy:

Health Insurance Portability and Accountability Act (HIPAA):
WMU is required by federal law to provide a Notice of Privacy Practices describing how health information that is maintained about you by the Western Michigan University Group Health Plan may be used or disclosed. http://www.wmich.edu/hr/ppm/chapter-topics.html#19
Freedom of Information Act (FOIA):
Requests for data from outside WMU must go to the FOIA officer.wmich.edu/gc/
Identity Theft Prevention

WMU Policy on Using Mobile Computing Devices
