Required forms

Please read through and identify what VA Forms are required according to the student's needs. Go to www.gibill.va.gov and click on Education Forms or Electronic Application Form to access the appropriate form(s). The student must fill-up the form(s) completely and send, together with applicable documentation, to the Office of the University Registrar, Western Michigan University, 1903 W Michigan Avenue, Kalamazoo, MI 49080-5256. Educational assistance for new students

If the student is the veteran and has never used the VA educational benefits before, the forms required are:

• VA Form 22-1990 or fill out online at VONAPP: Application for Education Benefits

• VA Form 21-686c : Declaration of Status of Dependents (if applicable)

If the student is the dependent and/or survivor of a Veteran, the form required is:

• VA Form 22-5490 : Application for Survivors' and Dependents'

Educational assistance for returning students

If the returning student wishes to change his/her program or place of training, and he/she is the veteran and has used the VA educational benefit before, the forms required are:

• VA Form 22-1995 : Request for Change of Program or Place of Training

• VA Form 21-686c : Declaration of Status of Dependents (if applicable)

If the returning student is the dependent of a veteran who wish to change his/her program or place of training and has used the VA educational benefits previously, the form required is:

• VA Form 22-5495 : Request for Change of Program or Place of Training (Survivors' and Dependents' Educational Assistance)
  Married students must, likewise, submit a copy of their marriage certificate.
