This page is for 2nd Tier Professional Development conducted during the
2002-2003 school year ONLY ***
When
requesting reimbursement for 2nd tier training that occurs during the
2002-2003 school year, the following documentation is required. Incomplete
requests will not be processed. PLEASE NOTE THE CHANGES IN THE FORMS
(slightly different from 2001-2002). The documents listed below are duplicates
of those mailed to all Teacher Leaders in July, 2002.
DEADLINES
FOR REIMBURSEMENT REQUESTS (NEW) 2002-2003
Please submit all of the required
documentation, according to the directions below, to the project office
by the dates below. A district may request partial reimbursement in January
or wait and request full reimbursement for the year in May.

When applying for reimbursement for 2nd
tier professional development conducted at the district level, please submit
each of the required forms below. If you have any questions about the procedure,

A signed attendance sheet
is required for each PD session. Each participating teacher

Teacher Leader Professional Development Logs |
Each Teacher Leader must
submit a training log containing all M3RP PD for the reimbursement
period. This log should document all mathematics content and pedagogy
professional development for the period, even that in addition to
M3RP 2nd tier training.
Teacher Leaders will be compensated as presenters only when actively
presenting during a session. If guest speakers or other Teacher
Leaders are presenting, then a non presenting Teacher Leader will
be compensated at the participant rate rather than the presenter
rate. (form available below)

Non Teacher Leader Professional Development Logs |
Each grade 6 - 8 mathematics
teacher must maintain and submit a training log for the reporting
period. This log should document all mathematics content and pedagogy
professional development for the period, even that in addition to
M3RP 2nd tier training. (form available
below)

District Invoice |
A completed district
invoice must accompany request for reimbursement. Project invoices
must come from the project
office. An invoice will automatically be mailed to the district
upon receipt of the district's Proposed Professional Development
Schedule and Current Roster (see forms above)

Resource request |
Reimbursement for resources
to support M3RP 2nd tier training require either:
1. Original receipts or
2. Detailed invoice on school letterhead with appropriate authorizing
signature.
Maximum yearly reimbursement
for resources: $500/district
