Forms

As a Master's student, before you graduate you must submit the following forms:

• Audit form (to be submitted to the Registrar's office along with your payment)
• Audit form to be approved by the Graduate Adviser

Every semester that you register for one of ME 5950, 6950, 6970, 7000, 7100, or 7300, you must fill the following form and submit it to the MAE department office:

• Department form for electing ME 5950, 6950, 6970, 7000, 7100, or 7300
• Request to extend the time to complete graduate degree program

The first time you register for ME 7000 (Master's students) or ME 7300 (Ph.D. students), you must fill the following two forms:

• CEAS Thesis/Dissertation Screening Policy
• Graduate College form needed for forming a committee (required before you register for ME7000 for Master's students or ME7300 for Ph.D. students)
• Graduate College form needed for starting to take thesis/dissertation hours (required before you register for ME7000 for Master's students or ME7300 for Ph.D. students)
