• Registering for EDLD Core Comps

• Creating Your Dissertation Committee
  All students must complete this form, listing the names of their proposed dissertation committee members and obtaining their signatures. Note, it is "okay" to distribute this form by fax or PDF to members as needed to obtain all signatures on a single form. The approval process for this form may take four or more weeks to complete.

• 3rd Committee Member Form (to seek Graduate Faculty Status)
  Also need the person’s resume/vitae.
  If your proposed third "outside" person is not a WMU faculty member with current graduate faculty status, then he or she will need to obtain "temporary associate graduate faculty" status. This is accomplished by completing this form and e-mailing it (along with that person's vitae) to your advisor or Diane Bourgeois at . The approval process for this form may take four or more weeks to complete.

• Dissertation Proposal Approval Form
  Students should complete this form and bring it with them to their dissertation proposal meeting with their full committee.

• HSRIB Approval Forms
  Within this link, students need to use two forms (as part of their HSRIB materials):   (1) complete the "Application for Initial Review," and (2) use the "Template for Protocol Outline" as you cut and paste pieces from your dissertation proposal to address specific information requested in that template. Also, look within "Sample Consent and Assent Forms" and use the "Template for Informed Consent" for interviews, and the "Anonymous Survey Consent" for first page of a survey. Note, an "on-line survey" version is being created by HSRIB.
