The Long Distance Service Request form is an Excel spreadsheet. When you click the link below, it will open the spreadsheet onto your computer's desktop. Please fill in the form, save it to your desktop, and then attach it to an email to LD-request@wmich.edu.
After you submit your form, you will be contacted within 7-10 business days, confirming your new service, your online account number, username and password.
