PPM Section 3 - W-4 forms 

All new University employees must complete a W-4 form (Withholding Exemption Certificate), in compliance with U.S. Department of Treasury. This form also provides Payroll with necessary tax and withholding information.

Procedure — W-4 forms

1. Supervisors should have new employees complete this form on their first day. W-4 forms are available from Payroll.
   
   
2. Completed W-4 forms are to be returned to Payroll. Also see Payroll Deductions.
