Information for benefits-eligible employees who are part of the Aviation Associates employee group.

Benefits and rates summary

The Benefits and Rates Summary PDF document is a three-page overview of your insurance benefits, associated rates, and other benefits including sick leave, leaves of absence, and holidays. This document is referred to often during the benefits orientation video. Please print a copy for your reference as you view the presentation and complete your enrollment forms.

The benefits orientation presentation gives instructions for downloading these same forms. The links are also provided here for your convenience. Please be sure to bring the completed forms with you to Human Resources.

  1. Health insurance enrollment form: MS Word Document or PDF Document.
  2. Notice of special enrollment form: Notice of special enrollment MS Word version or Notice of special enrollment pdf version.
  3. BASIC Flexible Spending Account enrollment form: PDF Documents.
  4. Employee handbook receipt form: Employee handbook receipt MS Word version or Employee handbook receipt pdf version.

All employees eligible for benefits must view the benefits orientation video presentation and enroll in benefits within the first thirty-one days of employment. Watch the video presentation.
