BASIC administers health care and dependent care reimbursement accounts for employees who opt to participate in these flexible spending accounts. The maximum annual contribution to a health care flexible spending account is $2,500. The maximum annual contribution to a dependent care flexible spending account is $5,000, or $2,500 for employees whose income tax status is married filing separately.
As of the 2014 plan year, health care flexible spending account participants may carry over up to $500 of unused funds into the next plan year, with the first carryover from 2014 to 2015. This option, announced recently by the U.S. Treasury, is a modification to the use-it-or-lose-it provision. Please note that it does not apply to the dependent care flexible spending account.

To enroll or re-enroll in a flexible spending account for 2014, complete the BASIC Flex Enrollment form, located in the last page of the booklet: PDF.
If you are not able to print the enrollment forms you may pick up a copy of the form(s) in Human Resources.
Submit your completed Health Insurance Enrollment and Change form and BASIC Flex Enrollment form to Human Resources no later than 5 p.m., Friday, Dec. 6, 2013.
Health care benefit and flexible spending account elections are effective Jan. 1, 2014. Payroll deductions begin with the paycheck received on Jan. 14, 2014.
