The University provides faculty and staff with a certain amount of group term life insurance and the option to enroll in additional life insurance.
Employees may enroll in the University provided life insurance and the optional, additional life insurance (coverage for themselves) during their first 31 days of employment (coverage is guaranteed) by completing a Life Insurance Enrollment & Change Form. Select the form for your employee group on the HR Forms page. Please return the form to Human Resources.
After the first 31 days, employees may apply for the optional, additional life insurance (coverage is not guaranteed) by submitting an Evidence of Insurability application to Standard Life, our group life insurance carrier. Please contact Human Resources for more information and an application.
You may change your beneficiaries at any time by completing a Life Insurance Enrollment & Change Form which may be accessed via the HR Forms Web page.

Coaches, Research Staff (grades R1, R2, and Q only), and
Staff Compensation System Exempt and Non-Exempt employees
who are enrolled in Additional 1 Life may also enroll in

Standard Insurance is the University's group life insurance carrier. For claims service, you can reach Standard at 1-800-628-8600. The WMU Group Number is 642075-A.
