
On October 1, 2005, The Center for Disability Services was awarded a three year grant from the Michigan Developmental Disabilities Council to create a model of healthcare coordination for individuals with developmental disabilities.
We will work directly with 50 individuals with developmental disabilities to help coordinate their health care needs. This could include assuring access to the doctor, assuring participation in preventative care and participation in health promotion activities.
The Healthcare coordinator or outreach worker can help you:
• Locate a doctor
• Attend appointments
• Solve problems related to your healthcare
• Help locate community resources
Any healthcare issues including:
Any adult in Kalamazoo County with a developmental disability.
