Teacher Survey Cover Letter

This survey is being administered to gain important information about you and your experiences as a teacher or staff member in a Pennsylvania charter school.  It is part of a self-evaluation by the school and one component of the initial study of Pennsylvania charter schools being conducted by The Evaluation Center at Western Michigan University for the Pennsylvania Department of Education.

All responses are confidential, and no responses will be individually identified.  A summary of the results will be returned to each school for use in the school’s continuous improvement process.  In addition, the survey results will be incorporated into statewide evaluation reports on the charter schools in Pennsylvania and other research reports.  Your responses will be confidential and used only for research purposes.

- Please also respond to the open-ended questions on the back side of this cover letter.

- After completing the attached survey and open-ended questions, seal them in the 
   envelope provided and return it to the designated person at your school 
   (_____________________________) and ask that your name be checked off the list.

- Your name is written on the return envelope only to assist us in following up with 
   those persons who do not initially respond.  Do not write your name on the survey itself. 

- Once all the surveys have been collected from the teachers and staff members at your 
   school, they will be forwarded to The Evaluation Center.  The Evaluation Center staff 
   will open the sealed envelopes and analyze the data.
