Students Survey Cover Letter

This survey is being administered to gain information about you and your experiences as a student enrolled in a Pennsylvania charter school.    It is part of a self-evaluation by the school and one component of the initial study of Pennsylvania charter schools being conducted by The Evaluation Center at Western Michigan University for the Pennsylvania Department of Education.

All responses are confidential, and no responses will be individually identified.  A summary of the results will be returned to each school for use in the school's continuous improvement process.  In addition, the survey results will be incorporated into statewide evaluation reports on the charter schools in Pennsylvania and other research reports.  Your responses will be confidential and used only for research purposes. 

º Make sure to use a No. 2 pencil when completing the survey and mark only one answer for each question. 

º Select answers based upon your own perceptions and experiences. 

º After completing the attached survey and open-ended questions, return them to the person who is administering the survey in your class. 

º Once all the surveys have been collected from the sampled students at your school, they will be forwarded to The Evaluation Center where the data will be summarized and analyzed.
