Instructions for Administering the Teacher Survey

All teachers and school personnel involved with instruction, including administrative and professional support personnel, will complete this questionnaire.

  1. A list of all teachers, administrators, and professional staff at the school should be compiled (see attached pages, which include a checklist).

  2. The questionnaire, together with the cover letter and an envelope, should be given to all the individuals included on the list. This can be done by placing the materials in their mailboxes, distributing them directly to the individuals, distributing them during a meeting of teachers, etc.

  3. The respondents should be asked to complete the questionnaire, enclose it in an envelope, and then return it to a designated person at the school.

  4. It is important that the respondents complete the surveys independently. Therefore, if the respondents are to complete the survey in connection with a meeting, it is important that they can work in silence.

  5. Teachers should not place their names on the questionnaire, although they will be asked to check off their name from a list when the questionnaire is returned, so that we can trace and follow up on missing respondents. Teachers may choose to place their names on the envelope.

  6. If one or more individuals could not complete the questionnaire and were therefore not checked off the list, please write a few notes listing their position and the reason why they could not complete the questionnaire.

  7. When all questionnaires are accumulated at the school, they can be returned to The Evaluation Center.  A copy of the checklist of the persons who were to complete the questionnaire should accompany the completed questionnaires.
