After completing the questionnaire, place it in the envelope provided, seal it, and return it to the designated person at your school (_____________________). While we ask that you not write your name on the questionnaire, we want you to be aware that your school's designated person is keeping a checklist of the persons receiving this questionnaire. This is necessary in order to follow up with teachers or staff persons who have not yet responded. When you return the questionnaire to the designate, make sure your name has been crossed off.
