Teacher and Staff Survey


This survey is being administered to gain important information about you and your experiences as a teacher or staff member in a Michigan public school academy (charter school). It is part of a self-evaluation by the school and an element in a statewide evaluation conducted by The Evaluation Center at Western Michigan University. The Evaluation Center has been commissioned by the Michigan Department of Education to evaluate the charter school initiative and NOT to evaluate individual schools. Therefore, while the results will be summarized and returned to each school for improvement purposes, The Evaluation Center will not share school-level results with other parties (e.g., authorizing agencies, Michigan Department of Education, media, etc.). Your responses will be confidential and used only for research purposes.

While we ask that you not write your name on the questionnaire, we want you to be aware that we are keeping a checklist of the names of the persons receiving this questionnaire. This will assist us in following up with persons not responding. In some cases, you are being asked to provide factual information about yourself, and in other cases about your perceptions of the school. After completing the questionnaire, return it to the designated person at your school (_________________________) and ask that your name be checked off the list.
