Student Survey Cover Letter
We are asking you to fill out this survey because we want
to know about you and your experiences as a student at your school.
Your school and the Connecticut State Department of Education need to know
whether or not you are learning as much as you can, are receiving the help
you need, and are enjoying your time at school. The Evaluation Center
at Western Michigan University has been hired to collect the information
about you and your school. You are being asked to provide information
about yourself and about what you think about your school. After
you have completed it, return the survey to the person who gave it to you.
Do not write your name on the questionnaire.
The results of the surveys filled out by all the students
from your school will be summarized and returned to each school.
In this way, the school will not find out anything about how you have personally
filled out the survey. In other words your answers will be confidential.
