Instructions for Administering the Parent/Guardian Survey
- A list of all students in the school should be compiled.
- Together with the evaluator, or representative of the evaluation team, randomly select 40 students from this list. If your school has less than 100 students, 30 students will be sufficient.
- These students should not be from the same families, since this would result in sending two or more questionnaires to the same family.
- An equal number of students should be selected from each grade level in the school.
- An attempt should be made to include students from as many different classrooms as possible.
- A checklist should be prepared with the names of all the students whose parents/guardians will be asked to complete a questionnaire. A checklist is included on the following pages.
- The selected students should be assigned to two groups of equal size. (If you use the list on the following pages this division can be done by selecting odds and evens.) The parent/guardians of the students from the first group will receive questionnaires during December 1997 or January 1998. The parents/guardians of the second group of students will receive similar questionnaires during May 1998.
- The questionnaires can be sent home with the students or delivered to the parents by another appropriate method (i.e., mail, handed directly to parents, distributed at school function, etc.). Each family should receive a cover letter, the questionnaire and a return envelope with the family name on it. All of these things will be provided by the evaluation team.
- One parent or guardian from each family selected should complete the questionnaire and return it to the designated person at the school.
- The parent/guardian should not write his/her name on the questionnaire, although their name should be written on the envelope. When the questionnaires are completed and returned, the name of the family/student should be checked off the list to denote that the questionnaire was completed and returned. The envelopes should not be opened at the school. A summary of the results for each school will be returned to the school after the data analysis is completed.
- The designated person responsible for this survey at each school is encouraged to follow up with nonresponding families. If the number of nonresponding families is high, we will have to re-sample the school.
- After seven to ten working days, the designated person at each school should return to The Evaluation Center (i) all the questionnaires, (ii) a copy of the checklist of names, and (iii) a list with the total number of students at each grade level in the school.
