The Evaluation Center at Western Michigan University has been contracted to lead a long-term evaluation of the Connecticut charter school initiative. This evaluation is a collaborative effort with the operating charter schools and the Connecticut State Department of Education, and The Evaluation Center. To ensure that the evaluation is comprehensive and includes the input and perspectives of essential persons like yourself, we ask that one adult parent/guardian/caregiver in each household complete and return the enclosed survey form. For your convenience, an addressed, postage-paid envelope is included to return the completed form directly to The Evaluation Center.
The information we obtain from the survey will be used for feedback to schools and as a part of the overall evaluation of the charter schools. The code number on the form will be used for follow-up with nonrespondents. However, your responses will not be identified with you in any way and you will not be named in any report. In other words, your responses will be treated in a very confidential manner. Only summarized data will be shared with the charter school and/or included in any formal reports to the Connecticut State Department of Education.
