Public School Academy Basic Information Form
This form is intended to provide a common set of information about Michigan
Public School Academies that can be used for statewide evaluation efforts
and for other purposes deemed useful at the school. It is intended as a
guide for reporting basic information about this Public School Academy/Charter
School and for developing a portfolio of important information about this
school.
This form/guide should be reviewed at the beginning of the school year,
and the requested information should be added as available and appropriate.
On a periodic basis, a representative of the external evaluator will communicate
with the school's administrator about the type and kinds of information
that might be included. We will not be prescriptive about a number of information
types. We fully expect that schools will find a variety of ways to respond
to the same question. On the other hand, there will be some information
that can be taken directly from other data bases. Finally, we hope this
guide/form will help organize information that may be requested by a number
of groups, and it could provide a strong resource bank for school newsletters
and/or school reports.
