
It is strongly recommended that you print out this list and check off each item as it is completed. Missing items will void your application.
1. A Baccalaureate degree from an accredited college or university
2. Completion of all Didactic Program in Dietetics’ requirements.
3. A minimum cumulative grade point average of 3.0.
4. Submission of the following materials to the Dietetic Internship Admissions Committee.
5. Participation in the D&D Digital computer matching program.
• A completed application form.
• A typed letter of application clearly stating (1) your short-term and long-term professional goals, (2) personal strengths (3) areas needing further development and (4) experiences desired in an Internship Program. ADA's official application form (see above link) requests that you list “experiences related to dietetics in the past five years.” However, in your essay you may also describe significant volunteer, paid, or life experiences you have had, before or after 2003, that demonstrate ability to succeed in our internship program. This might include (but is not limited to) permanent full-time employment or raising a family. If you are competent in a second language, please report that, too. Your letter is an important component of the selection process. You may include a resume along with your letter of application.
• An official transcript from each college and university attended. Sealed transcripts stamped "issued to student" across the back seal will be accepted.
• A signed Verification Statement or Intent to Complete form from your advisor.
• Three professional references. Two should be from instructors in the major and one from a work supervisor. Include waiver forms for each recommendation. Recommendation Form
• A $35.00 non-refundable application fee. Check or money order should be made out to the Department of Family and Consumer Sciences.

Kalamazoo, MI 49008-5322Applicants will be ranked on a point system for acceptance into the program since only a limited number of applicants can be accepted each year. All submitted application materials and admission deliberations are strictly confidential.
1. Apply for admission to the Western Michigan University Graduate School in the Department of Family and Consumer Sciences. Students with a grade point average below 3.0 are admitted with probationary status. This status means students must earn grades of B or higher in graduate courses to remain in the Program. Application to the Graduate College can be made after the applicant has been accepted into the Internship Program
2. Sign and return the Agreement of Understanding, which will be provided by the Program Director after acceptance into the program.
3. Provide results of a complete physical examination, including a TB test, rubella titer, Hepatitis B vaccination and proof of health insurance coverage, to the program director before start of program.
4. Provide evidence of Hepatitis B vaccination and proof of health insurance coverage to the program director.
5. Pay tuition and fees for the Dietetics Internship program and for graduate coursework (15 credits the first year). The bills for first semester are usually sent out after Labor Day.
The internship start date for 2009 is August 31st.
