Advance Reservations for Instructional Videos

IMPORTANT MESSAGE TO PATRONS:

This form is intended for WMU Main Campus users only. Users at WMU regional sites should request videos through Interlibrary Loan .
The Instructional Video Collection is now housed at Video Services in Waldo Library, Patron Services Window (First Floor).
Requests need to be submitted at least 48 hours in advance of pickup date (excluding weekends) by current faculty, staff or graduate assistants. If your request cannot be processed as submitted, we will contact you by telephone or email within 48 hours (excluding weekends).
