Advance Reservations for Instructional Videos

IMPORTANT MESSAGE TO PATRONS:

This form is intended for instructors on the WMU Main Campus .
Requests need to be submitted at least 48 hours in advance of pickup date (excluding weekends) by current faculty, staff or graduate assistants. If your request cannot be processed as submitted, we will contact you by telephone or email within 48 hours (excluding weekends).
Instructors at WMU regional sites should request videos through the Interlibrary Loan System at least 4 working days prior to the date needed. Clearly request advance reservation dates in the Notes field of the Audio/Visual Request page. These videos will be shipped to your home address. You will be notified by telephone or e-mail if your request cannot be fulfilled.
