The U.S. Bureau of Citizenship and Immigration Services (BCIS) requires evidence 
of adequate funds to meet the financial obligation of enrollment at the 
Career English Language Center for International Students (CELCIS) at 
Western Michigan University for issuance of an I-20.  Please complete 
this form in full using US dollars when listing funds.  Answer all 
questions that apply to you.  If you are completing Section B your 
sponsor must attach an official bank statement indicating the amount of 
funds available.  If you are a scholarship student, send a current award 
letter which includes length of support and billing address.  Your 
signature in Section D is required.
